Coordinating Resources and Planning to End Homelessness

What the CoC Does

The Continuum of Care (CoC) is composed of representatives of relevant organizations that are organized to plan for and provide, as necessary:

  • a system of outreach, engagement, and assessment
  • emergency shelter
  • rapid re-housing
  • transitional housing
  • permanent housing
  • homelessness prevention strategies

The CoC is responsible for coordinating and implementing a system within its geographic area to meet the needs of people experiencing homelessness and their subpopulations.

Primary Responsibilities

The CoC’s primary responsibilities under the HEARTH Act include the following:

  • Operating the CoC
  • Designating and operating a Homeless Management Information System (HMIS)
  • CoC planning

In Columbus and Franklin County, Ohio, the CoC stewards all strategies developed under A Place to Call Home and A Place to Call Home for Youth (frameworks for action to address homelessness in Columbus and Franklin County). The CoC provides funding for housing capital, services, and operations in Columbus and Franklin County, coordinates activities across frameworks, promotes collaboration to achieve goals and strategies, and secures resources for programs and projects.

Annual Plan

Driving Results in Franklin County

The annual plan for the Columbus and Franklin County Continuum of Care (CoC) serves as the strategic framework for coordinating homelessness services and federal funding. The plan outlines the critical responsibilities and monthly milestones for the CoC, the CoC Board, and CSB as the lead agency.

System Oversight and Governance

Managing the selection of the CoC Board, reviewing membership, and ensuring progress on Diversity, Equity, and Inclusion strategies.

Strategic Planning and Funding

Coordinating annual funding allocations for ESG and CoC programs, securing resources for new permanent supportive housing, and monitoring compliance with the HEARTH Act.

Data-Led Performance

Operating the Homeless Management Information System (HMIS) and conducting the annual Point-in-Time count to ensure a housing-focused, data-driven response to homelessness.

Community Collaboration

Aligning local activities with the "A Place to Call Home" and "A Place to Call Home for Youth" frameworks to achieve lasting results for our neighbors.

HUD Continuum of Care Application

FY2025-26 Notice of Funding Opportunity

  • The FY25 Notice of Funding Opportunity (NOFO) has been temporarily rescinded due to a court injunction. As a result, the FY25 NOFO is not currently available, and applications are not being accepted. We are actively monitoring the situation and will provide updates as soon as new information becomes available. Please continue to check this page regularly for the latest announcements.
The CoC Board met on September 12, 2022 and the full CoC met on September 20, 2022 to review the 2022 CoC Application and rank Project Applications according to the 2022 CoC Review, Score, and Ranking Procedures. Please see below for the CoC Consolidated Application, CoC Priority Listing, and the CoC's decision for the ranked position of the project applications. The CoC accepted all projects but one. No projects were reduced. No renewal projects were placed in Tier 2. Renewal YHDP, CoC planning, and UFA projects were accepted and not ranked, per HUD’s guidance.
The CoC Board met on October 25, 2021 and the full CoC met on November 4, 2021 to review the 2021 CoC Application and rank Project Applications according to the 2021 CoC Review, Score, and Ranking Procedures. Please see below for the CoC Consolidated Application, CoC Priority Listing, and the CoC's decision for the ranked position of the project applications. The CoC accepted all projects. No projects were rejected or reduced. No renewal projects were placed in Tier 2. Replacement YHDP, CoC planning, and UFA projects were accepted and not ranked, per HUD’s guidance.
The 2020 Continuum of Care competition was replaced with a non-competitive renewal process, per the 2021 Consolidated Appropriations Act.
